Functional Medicine is a personalized, systems-oriented model that empowers patients and practitioners to achieve the highest expression of health by working in collaboration to address the underlying causes of disease.

The Functional Medicine Exam includes:

 

  • Comprehensive medical, social, and surgical history review and physical exam performed by a Elements Holistic  internal medicine specialist and referrals to sub- specialist as necessary

  • Review and management of medications and immunizations

  • Lifestyle assessment to discuss approach to nutrition, stress management, sleep, alcohol, tobacco and drug use, personal safety, happiness and other indicators of disease risk

  • Test results thoroughly explained directly to the patient by the physician at follow up visit

 

The Functional Medical Exam may include any of the following tests based on the patient’s medical history, physical exam, and physician’s recommendations.
